Forms available Online for the Application Process:

1. Download the PDF format documents listed below
2. Print out the documents and fill them out completely, following the directions on the forms.
3. Mail the Application, Medical, and Emergency forms to the College. Make sure you read the questions on the application form and that you fill it out completely including your signature. Make sure you include all the items listed on the application form at the time of your application.
4. Include the Application fee of $35 (US funds) with your application
5. Give or mail three Reference Forms to non-family members with one form for your pastor. All forms are to be returned directly to the college office and NOT given to the prospective student.
6. Fill in and mail the Transcript Request Form(s) to the applicable school(s).
